REQUEST FOR MEDICAL FILES

We are happy to provide files for insurance, job or other medical-related reasons. We will only release these files to you or your designee. Due to confidentiality, we will only accept requests from recognized insurers or yourself. If you wish copies of your files, you must visit our offices in person and sign a file release form. Your files will then be mailed to you or your designee.

There will be a fee charged for copies of records and x-rays payable prior to duplication, and the patient must sign for a release of records. Records will not be duplicated without prior payment.
